An otherwise healthy 17-year-old woman presented with a six-month history of persistent and pruritic lesions on her lower back. Physical examination showed well-circumscribed, brownish-black pigmented patches overlying the protuberant spinous processes of lower thoracic and lumbar vertebrae (figure 1A). The patient was a student with a lean body and she spent most of her time lying or sitting down against a rigid backrest.
