A 52 year old Caucasian man with a 10-year history of HIV infection presented to the Infectious Disease Unit with a 3 week history of diffuse itch, cough and dyspnea. Topical steroids given over 1 month and pyrethrine over 3 days, administered ex adjuvantibus, were ineffective.
