Patient education in heart failure: feedback from Antoine Beclere hospital

Summary : Heart failure (HF) is a chronic illness characterized by a heart not able to ensure an adequate blood supply. It is common, severe and costly. HF is associated with frequent rehospitalizations, poor quality of life and loss of autonomy for the elderly. It is therefore a major public health problem. The management of HF is multifactorial and includes therapeutic patient education (TPE). It is a continuous process, focused on the patient, integrated care and recognized since July 2010 by the “Hospital, Patient, Health, Territory” French law. This article describes an experiment in establishing a TPE program for patients with HF in the cardiology department of Antoine Beclere's hospital. The preliminary phase includes a review of the literature, an inventory of existing educational actions and an assessment of the needs of the educational team. The design of the TPE program includes different educational sessions: from educational diagnosis to evaluation. Learning and assessment tools are also described. The first difficulties encountered in the implementation of the program are primarily patients recruitment, patients follow-up, consultations planning, and communication about the project. For each of these challenges, improvements have been considered. The main assets of this program are the motivation of the educational team, the structure of the program, the content of the sessions which are tailored to each patient, and the encouraging results. Finally, this TPE project allows pharmacists to take their clinical role back into account.
