Improvement in the results of renal transplantation over the last 10 years

Summary : Over the last 10 years, renal transplantation has suffered from a lack of donors. In 1999, in France only 1 842 patients out of 5 879 registered on the waiting list, including 2 485 new patients, were transplanted. In the 1980's, ciclosporine greatly improved the prognosis of transplantation. To date, new, more selective, less nephrotoxic and less cytotoxic immunosuppressive drugs have become available and have decreased by 10-15% the incidence of acute rejection. These drugs, associated with therapeutic improvements in anti-infectious treatments, have increased renal and patient survivals. However, in the long term, the occurrence of cancer, especially lymphoma, is frequently reported. In the near future, several ways of increasing the number of grafts will become possible: by educating the population, by increasing human and material ability of organ-harvesting centers, and transplantation from living donors and older or non heart-beating donors. The development of xeno-transplantation is not anticipated in a near future, so it is likely that the waiting list will continue to grow.
