Almost one third of patients with non-small cell bronchial cancers are operable.
Over the last ten years, the management of these patients has changed radically: while surgery remains the cornerstone of treatment, initial large-scale trials have shown the value of adjuvant chemotherapy and induction treatments are sometimes discussed prior to surgery.
In addition, radiotherapy helps control the spread of some of the more aggressive operated forms on a loco regional level. In all cases, the aim is to enhance the cure rate of this cancer through a pluridisciplinary approach to its management.
Other areas, particularly diagnosis, have also undergone substantial change: the WHO pathology classification system has been modified, FDG PET is in widespread use and new sampling methods are available.
The biological mechanisms underlying bronchial cancer requires further elucidation and this should allow current classifications to be transcended, leading in the future to improvements in the defining bronchial cancers at high risk of recurrence, the indication and modalities of adjuvant treatment and finally the type of follow-up to be proposed.
In parallel, the place of targeted molecular therapies will also need to be discussed in the near future.
