A better understanding of the fundamental mechanisms of carcinogenesis has led to the identification of the key changes inherent in tumour phenotype and to the development of targeted molecular therapies, for which the agents used are specifically evaluated for their ability to act on a molecular target implicated in carcinogenesis.
Several potential targets have been identified and several agents have already demonstrated their clinical activity, justifying the marketing authorisations granted for the management of gastrointestinal stromal tumours, and cancers of the breast, lung, upper respiratory tract and colon, etc., while new molecules are already being studied in the lab or have entered the early clinical phase.
This book provides a precise inventory of already authorised molecular therapies and those still in the pipeline.
The major mechanisms involved in the onset of cancer are thus considered, ranging from the activation of membrane receptors to the deregulation of nuclear processes (cell life cycle, epigenetic regulation, control of mitosis and senescence), and including cytoplasmic transduction routes without neglecting to analyse the essential interactions between tumour cells and their environment (angiogenesis, stroma remodelling).
The authors review the molecular basis for each of these mechanisms and describe the arguments put forward in favour of therapeutic targeting.
The existing agents are precisely analysed, giving in detail the preclinical data and above all the results obtained during therapeutic trials, in terms of both efficacy and toxicity.
Finally, as research in this field is advancing extremely rapidly, the future perspectives are discussed.
